Israel Nachfolger, RN, ASN, BS, NR-EMTP

Owner & CEO

Israel brings a unique mix of clinical and operational experience to Pinnacle, having worked in healthcare as both a practitioner and in various administration roles for over a decade. In addition to his role as CEO, Israel continues to practice as a Registered Nurse and volunteer Paramedic while managing multiple Skilled Nursing Facilities on the East Coast.
Israel lives in the foothills of the Ramapo Mountains in the Hudson Valley in Monsey, NY with his wife, Miriam, and 6 children. An avid lover of music, Israel enjoys honing his skills as a musician at the piano and practicing his passion as a recording engineer in his free time.

Anne Ambler-Cote, RN, BSN, LNHA

Administrator

Anne has worked in the field of long-term care for nearly 30 years, most of them at Pinnacle Sanford, where she has served in a variety of capacities.

Anne started at Pinnacle Sanford in 1988 as a Resident Assistant and became a Certified Nursing Assistant the same year. In November of 1989, she became a Geriatric Nurse Assistant in Pharmacology (now known as a CNA-Medication technician). She then received an Associate’s Degree in nursing from the University of New England in 1994 and continued working at Pinnacle Sanford as a Nurse Manager. She also worked at Pinnacle Sanford as the Director of Nursing Services for 10+ years.

Anne received a Bachelor’s Degree in nursing from the University of Southern Maine in 2011, she began the 6 month Administrator in Training (AIT) program at Pinnacle Sanford in September of 2012 and obtained her Administrator’s license in June of 2013, she has worked in the latter capacity at Pinnacle Sanford ever since.

Kristi Bouchard

Administrative Assistant

Kristi started working in long term care as a Talent Acquisition Specialist in 2016. Her experience includes working closely with a verity of nursing home facilities over several states helping to build their professional care teams.

Kristi grew up in Wells, Maine and enjoys traveling, crafting and spending time with her family, friends and two cats Poppy and Snickers. She is looking forward to her future with Pinnacle Sanford.

Lisa Benevento, RN

Director of Nursing

Lisa is a Registered Nurse who has worked in long term care for over 25 years in many different capacities. Lisa received her certificate as a Licensed Practical Nurse from Essex Technical School in 1994. Lisa graduated from North Shore Community College with an associate’s degree in nursing in 1999. Lisa is also a certified hypnotist. Lisa loves the personal relationships she builds with residents, families and colleagues. Lisa has been a Director of Nursing since 2007 and she loves her work.

Lisa grew up in Massachusetts on the North Shore and moved to Maine in August 2021.

Lisa has two adult children (they live in Massachusetts and New Jersey). Lisa is engaged to be married in 2022. Lisa’s hobbies include travel, gardening and yoga.

James MacPhee, PTA

Director of Rehabilitation

15+ Years of Service. Jim has been with Pinnacle Sanford for 18 years and is currently our Director of Rehabilitation.

Jim attended UMASS Amherst and Bay State College.

Hannah Swart, LSW

Director of Social Services and Admissions

Hannah started her career in long-term care in 2016 as a CNA here at Pinnacle Sanford and worked at both Pinnacle Sanford and North Berwick through college. Hannah obtained her bachelor’s degree in social work from the University of Maine in 2020. She then worked as a social worker in another long-term care facility and is very happy to be back at Pinnacle Sanford in her new role.

Marc Dupuis

Director of Environmental Services

Marc is a former native of Manchester, NH graduated from college in 1988 with a Degree in Business Administration. He began his career as a Loan Officer in a local credit union. After a 3-year term, Marc moved on to the public sector managing a small ski area, public skating rink and 5 public pools. Marc, not a fan of City Politics moved on to Coca Cola where he spent 20 years working in various positions and retiring in 2015. After taking a year off Marc went back to work in Sales for John Deere for 3 years, was a board director for the Black Bear Condominium Association who then hired him to be the General Manager of a 108 unit Condominium/Hotel complex.

Troy Fields

Food Service Director

Troy has over 25 years as a Food Service Director in the area. He was also Pinnacle Sanford’s FSD back in the 90’s.

Troy has degrees in Culinary Arts, Dietetics as well as Communications.

Sharon Dean

Activity Director

Sharon has over 12 years’ experience working as a certified Activity Director. Her experience includes long term care, adult day health and social day programs for the elderly. Sharon holds her certification through the National Certification Council for Activity Professionals and has an associate degree in Early Childhood Education through Cape Cod Community College.

Sharon grew up in Massachusetts and raised her son on Cape Cod. Sharon enjoys thrift stores, yard sales and riding her ATV in the woods of Maine.
